Service: Services
Description:

We are a non-profit organization locally managed and governed by a volunteer board of directors comprised of community members.  Our sole motivation is to provide hospice care regardless of ability to pay, age, diagnosis, code status, disability, race, religion, or gender.

  • The hospice interdisciplinary team for each patient includes their physician and/or our medical director, a nurse, certified nurses aide, a social worker, and a chaplain.  The team develops an individualized plan of care with the patient and their family.  We encourage active family participation in any decision made in the care provided.  Volunteers are included to provide companionship, emotional support, and respite care as needed.
  • Special services: Educational opportunities for community free of charge, memorial services, bereavement program.
  • Groups:
  1. Heartlinks: counseling to families with life threatening illness.
  2. Grief support groups.
  3. Compassionate Friends: for parents of any age who lost a child of any age to death. 
  4. New Day: for men and women going through the stages of grieving because of a death of a spouse or family member.
  5. TAG: a teenage group offers support to teens going through grief.
  6. Kids Time: for children ages 6-12 years that have lost a loved one.

Charges:

Support groups free.  Other services vary on a sliding scale.


Eligiblity:

Support groups are open to all.
All other services are for: St. Clair, Madison, Monroe, Clinton, Randolph, and Washington counties.


Insurance:

Medicare, Medicaid, plus others.
